Oral Medication
Medication may be stored in the clinic for your student.  York County requires parents or guardians to bring medicine to the clinic and complete a permission form. (Medication Permission Form ). All oral medication, whether over-the-counter or prescription, must be kept in the clinic and administered by the nurse. 

**Medications left in the clinic at the end of the year are discarded per YCSD guidelines. Please pick up medications prior to the last day of school. 

Asthma Inhalers
Asthma inhalers may be carried with students for self-administration once the physician’s permission form is completed:(Asthma Action Plan). Please send the completed form to the clinic.

EpiPens
EpiPens may be carried with students for self-administration once the physician’s permission form is completed: EpiPen Permission Form . EpiPens stored in the clinic and not retrieved before the last school day will be discarded at the end of the school year per YCSD guidelines.

PE Notes
Parents may write a note excusing their student from PE for up two classes. A physician’s note is required when lack of participation exceeds two PE classes.
